The
following items/information is needed along with
the product to be returned.
-
RGA
number that is received from IMTEC Corporation.

The
part number and lot number of item(s) being
returned. (If possible, attach a copy of the
invoice as it has this information on it.)
Used
/ Placed Implants
-
X-rays
taken at the time of the procedure or post-op.
If these are not available, then pre-op
x-rays are necessary.

The
completed implant questionnaire. To download
the questionnaire, click below
Procedure
- Assemble
all the items required in a secure package.
- Clearly
write the RGA number on the OUTSIDE of the
package. Please make sure that the package
is not in the form of an envelope, as they are
easily damaged due to postal machines.
- Mail
the entire package to the following address

For
all products being returned that is not subject
to an investigation:
- Product
will be authorized for return within 30 days of the date of
the invoice.
- A
15% restocking fee will apply to all returned product.
